Avoid retry on 'duplicate index name' index creation error

Description

In recent versions of Couchbase Server, an index creation race can result in the error "Create index cannot proceed due to presence of duplicate index name."

This should be added to the criteria used to return ErrAlreadyExists in CreateIndex.
