Logs missed when attaching logger

      Description

      Due to the nature of the logging in libcouchbase and the fact that a logger is not specified to lcb_create, there are log messages that are written by lcb_create which do not show up in the logger.  LCB should update lcb_create_st to contain a logger instance to allow all log messages to be captured by the application and/or higher-level SDK.
