Closed session object might survive rebalance and avoid being restarted

Instead having sessions restarting themselves it is more reliable to let them remove from the bucket, and notify it to go through current config and ensure that all endpoint have session associated. In this we don't need to know the indexes in the configuration, which also should fix CXXCBC-321.
