PHP unable to load dynamic library

I followed the instructions for installing on Windows (with one minor exception, I put extension=php_couchbase.dll because there is no "couchbase.dll" file in the download package).

I pasted the code example into a hellocouchbase.php file and I get an error message: Unable to load dynamic library 'php_couchbase.dll'

I'm not sure what I did wrong, and I'm also confused about what to do with the "libcouchbase.dll" file and the "package.xml" file that were in the download. I put libcouchbase.dll into the php folder (which is also in the system path), but I don't know if that's the right thing.
