Amend monitoring docs for neo

We have a couple of new features in neo

1- 4 more qualifiers:

• "results" - log a request if the number of results exceed this number
• "size" - log a request if the size of the results exceeds this number
• "mutations" - log a request if it has made more than this many mutations
• "counts" - log a request if the number of documents processed by certain operators (fetches, scans, sorts, joins) exceeds this number

2- collections of plans as part of system:active_requests and completed_requests is no longer tied to the request or server "profile" settings: we process the timings into a document as soon as the request completes (making then much less memory intensive but more cpu intensive) and collect them every time.
