Operator upgrade commands are wrong

The documentation recommends running the following commands to upgrade the operator:

Kubernetes

kubectl apply -f cluster-role.yaml

OpenShift

oc apply -f cluster-role-sa.yaml

These look like they are from pre-1.2 when we used cluster roles for the operator, as opposed to today where we just use roles.

We should really be discussing how to do the migration from cluster roles -> roles rather than just giving commands because I don't think they really fit in this context.
