Allow using connector name in offset storage namespace

This name should be part of the offsets of the source. Currently offset partition identified by bucket name and couchbase partition (vbucket). This new connector name should be also part of the offset partition.

When the name wasn't specified during configuration, the connector should generate and assign string UUID instead.
