Duplicate Indexes with the same name in a cluster

    Description

      It so happens that if the Index node (on which a particular index is created) goes down and if we create a new index with the same name, it is successfully created on another index node that is available at the creation time. Now, if the previous node comes up, it successfully rejoins the cluster and we have 2 indexes with the same name.
