Reduce the chances of log-flooding by connection pool errors

We can reduce the chances of flooding the diagnostic log with error messages by picking up on a repeating message and suppressing duplicates where we obtain a connection from the connection pool.

This would be limited in scope/impact (and therefore risk) but should still improve the overall utility of the diagnostic log.
