Cause for "index not found" not reported.

MB-28243 defers error reporting and so doesn't report a cause for "index not found" (code 12016).

It is arguable that this is redundant information - it is just repeating that the index is not found - and that the error code should be used, not the error text.

This will add the cause back to error 12016.
