[CLI] Transfer related tools should correctly handle non-default ports and alternate addressing

What needs to be done?
Right now we have a number of ports' related constants hard-coded in couchbaseConstants.py that are used in various 'pumps' functions. Since these ports can be re-mapped, we need to move from using constants to retrieving required ports from the cluster itself.
