moxi docs unclear on how to specify multiple Couchbase hosts

It's quite hard to find out how to supply multiple Couchbase server URIs to moxi, both on the command line and in the configuration file.

It would be great to have a few examples that actually demonstrate it.

In the config file, one must use vbar (|) for a separator instead of comma. That's documented but easy to gloss over, whereas an example would make it obvious.
