Exception in flusher loop: Unhandled case in sqlite-pst: 11 (database disk image is malformed)

Install couchbase server 1.8.1-871 on 12 windows server 2008 R2 64bit
Load 40 million items with size from 70 bytes to 512 bytes to cluster.
Reboot the whole cluster, there are 2 nodes (12 and 76) failed with error Exception in flusher loop: Unhandled case in sqlite-pst: 11 (database disk image is malformed)
