Add stream opaque field to close stream message

We need to add a stream opaque field to the close stream command in the extras section. This field will be used to specify the opaque number that the stream is actually using. The opaque field in the header will be used in the response for the closed stream. This is needed so that we can make sure that a close stream command is for a specific stream. Please also update the documentation in the cbupr repo.
