ChannelConnectionPool doesn't recover from a node outage

    Description

      The experimental ChannelConnectionPool is not successfully recovering from an outage or the loss of connections.

      Steps to reproduce:

      1. Setup a single node cluster
      2. Configure the application to use the channel connection pool experiment
      3. Loop and read a document every 5 seconds
      4. Stop the Couchbase Server
      5. Restart the Couchbase Server
      6. Observe that, unlike DataFlowConnectionPool, the operations never recover
