Php client should set its version in the HELO command

    Description

      The memcached.log will now print the output of the helo command, this is extremely useful as it allows Admins to see what clients are connected to the cluster:

      2016-04-05T13:43:34.121041+01:00 NOTICE 43: HELO [libcouchbase/2.5.7] TCP NODELAY [ 127.0.0.1:61904 - 127.0.0.1:11210 ]

      To get the most use out of this we need to ensure that the clients that are built on top of libcouchbase past this information down.
