CollectionQueryIndexManager should raise InvalidArgumentException when scope_name or collection_name options are set

From the RFC:

It's a cleaner solution for this than the scopeName and collectionName parameters added to QueryIndexManager option blocks. Those should now be deprecated. If either is used with any method on CollectionQueryIndexManager, an InvalidArgumentException must be raised.
