SDK3: Make most ops treat EXISTS error as CAS mismatch instead of DocumentAlreadyExists

As a result of recent changes to centralize error handling, most ops now treat EXISTS as DocumentAlreadyExistsException.  But this only makes sense for inserts, generally this error indicates CAS mismatch.
