Use streaming instead of / alongside heartbeat loop

Currently cluster information is updated once per minute by the heartbeat loop, which means that information in the CBMM-derived dashboards can be out of date by up to a minute.

One possibility for improving this would be to use ns_server's streaming APIs, which will push topology updates to us - perhaps performing a full heartbeat run if the streaming connection is lost.
