Description
1. The upgrade matrix stops at 4.0, and probably should be "4.x+" since there's nothing in there 4.0 specific.
2. The last sentence "Services require more planning during an upgrade." is super vague. Services is a reference to MDS, especially GSI. I recommend adding the acronym MDS to that sentence and also adding a link to the section "Multi-Dimensional Scaling (MDS)" on the Upgrading Couchbase Server page (https://developer.couchbase.com/documentation/server/4.5/install/upgrade.html)
