Fix formatting in query documentation

As a user, I want documentation to be correctly formatted so that headings and example files are clear and easy to follow.

Headings should be arranged in a heirarchy with less important headings being smaller.
The underlying markup should be correct and should not be visible in the output.
Links and cross references should work.
