Step by step guide for setting up certificate-based authentication

Our current documentation on setting up certificate-based authentication is dispersed in several different places, e.g. server-side, client-side. This is to track the need for a step-by-step guide of setting up certificate-based authentication from cradle to grave, including examples that are not limited to self-signed certificates generated with openssl.
