Improve overview of CBO

Bring CBO documentation inline with documentation for established, relational DBs.

1. Better introduction, including the purpose of the optimizer.
2. Emphasise that users need to update statistics on the Cost Based Optimizer page.
3. Explain how the optimizer fits into query plans.
4. Update diagrams on the Cost Based Optimizer page.
