Improve error handling docs with how to handle details

Current documentation is quite good on error handling, but can be improved in how to interpret classifications and details. This is mostly under the covers in the sdk-rfc and we may want to refer to the sdk-rfc, but need to be careful about the public interface definitions.

As a matter of fact, this is mostly about defining what is public and what context may be in errors that is volatile, but useful.
