Source: Optionally publish ignored events to "black hole" topic

Add a `couchbase.black.hole.topic` source connector config parameter. If non-blank, for each Couchbase event ignored by the Filter or SourceHandler, a tiny record is published to this topic just so the Kafka Connect framework will persist the source offset associate with the event.
