Support for duplicate / replica GSI index creation

For the purposes both of load balancing and HA, we need the ability to easily create multiple indexes with the same definition. While this can be accomplished today by manually specifying the node name, it would be better to allow for automatic creation and placement of any number of indexes without being aware of the topology.
