connection pool size incorrect if number of target nozzles is changed

Currently xdcr creates a connection pool for each pipeline, and sets the size of the pool in proportion to the number of target nozzles. If the number of target nozzles is changed, e.g., due to settings change or due to topology change on the target, the pool size does not change in accordance and ends up being incorrect.
