Query monitoring metrics should be aggregated

Depending on "where" the aggregation is to be done, this is either a query issue or an ns_server issue.

At the moment, any output from our newly added query monitoring/metrics is only per-node which will be a little confusing to users who are trying to see all of the requests (completed/active) across a cluster of potentially many nodes running the query service.
