Indexer logging improvements

Some log improvements are needed:

• During recovery, stream reader is going to log a ton of

"MutationStreamReader::checkAndSetBucketFilter Missing bucket default in Filter for Stream MAINT_STREAM"

as the bucket has already been cleaned up. This message needs to move to Debug level.

• It would be good to log an error message if vbuuid 0 is received for any vbucket. This happened in one of the customer scenario. It is better to flag it as explicit error.
