Remove LWW code that isn't going to be supported

1. Remove adjusted-time/drift code.
2. Remove per document conflict resolution mode.

None of this code is supported, so remove the code so that the team aren't burdened by it for any other development/mainenance.

This also reduces document and hashtable storage usage by 1 byte per document.
