Can't distinguish a failed add from out of memory

    Description

      Right now when loading data off disk we use add for when we are allowing sets to come in asynchronously. However, we consider a failure to mean we're out of memory, meaning that duplicated keys on disk will cause memcached to crash.

      Need to either fix this or not duplicate items on disk, preferably both.
