the 'gateway' proxy should handle a null default bucket

    Description

      To support default bucket deletion, moxi should support running a 'gateway moxi' which will behave correctly when the downstream does not have a "default" bucket defined, meaning the vbucket/membase process does not do anything with unauthenticated requests.

      Note that ns_server has had some changes to support this already by always including default in the :
