Monitoring / Profiling: some namespace cleanup needed

With profiling and clustered monitoring a few things got moved around, so now for instance Docs in execution are no longer Docs but Items, and completed_requests has moved from accounting to server, with the net result that there are still references to the completed_request_log (for instance in datastore/system) and there is confusion as to which method in server might be relevant to the active requests and which to completed requests.
This is to keep track that such cleanup is needed.
