topology change detector needs to collect info from the same target node

As of now topology change detector could connect to target node more than once.
1.     Get cluster id from a node
2.     Check if cluster id == cluster in repl spec
3.     If cluster match, get bucket list from the node

There is an issue that the multiple connections may be made to different target nodes, and the cluster id check would be ineffective in this case. this needs to be fixed
