[CX] Handle Master<-->Replica Network Drops

Currently, if a partition master loses network connection with a replica node, the replica will be disconnected. If this network connection is detected by the CC due to heartbeat misses, the replica node will be reconnected when it joins the cluster again. However, if it isn't the case, the replica will stay disconnected. We should either create a new replica when this happens or detect the network connection is restored and reconnect the replica.
