Log more information if index planner fails to satisfy constraints

During rebalance, index planner checks if the various types of constraints are satisfied by the proposed index plan.

If the constraints are not satisfied, it is possible that the lost replicas remain lost during rebalance.

There is need for better logging. Especially, which constraints were not satisfied and why, so that debugging process will be faster.
