7.0: Flaky Add Bucket UI Behavior

The first time I tried to add a bucket, the Add Data Bucket dialog appeared but all the settings controls never appeared (I did not think to take a screenshot).

The second time I tried to add a bucket, the Add Data Bucket dialog appeared correctly. However, the controls did not behave correctly as you can see in the attached screenshot (all 3 types were selected). I quit Couchbase Server.

The third time I tried to add a bucket everything worked as expected.
