Duplicate index creation when watchers are not active

Duplicate indexes can be created if we create index with same name when the watchers to indexers with the existing index are not active

Steps to repro
1. Create a 5 node cluster n0:kv+n1ql,n1:index,n2:index,n3:index,n4:index
2. Create index idx_name using with clause on n1 and n2
3. Restart the query process
4. Query process was instrumented to add delay while connecting to n1 and n2 on restart
5. After restart create index with idx_name but without the with clause now
6. We can see indexes getting created on n3 and n4
