Log document key > host mapping to help identify server issue

    Description

      It would be useful to log a document key > server mapping to help identify if one or more nodes in a cluster contain keys that are unavailable.

      The log entry needs to be at a low level not to flood logs during normal operation. An alternative option would be to append the node hostname to the failed operation exception as a parameter.
