DOCS - Enhance Error Handling Page

Rewrite the Error Handling page of the PHP SDK docs; at least the Retry section. It does not make sense to recommend doing a manual retry loop since the libcouchbase will itself do retries. It also seems that not all operations failures may result in retries, so that must be properly documented (libcouchbase 3.1.0).
