Include Cluster UUID and Cluster Name in CAO Metrics

To allow customers to correctly identify clusters in large deployments, we should add the Cluster UUID (cluster_uuid) and the Cluster Name (cluster_name) as labels in any metrics that are cluster-specific.

Cluster UUID can be retrieved once the cluster is initialised (or on first connection on CAO startup).

Cluster Name is mutable, but in a CAO-managed cluster, can only be changed via the CouchbaseCluster CR, and defaults to the same as the name of the Kubernetes resource.

Both the existing name (from the resource), and cluster_name (as in the actual name of the cluster), should be output, even if they are the same.
