Update esaml code in ns_server

ns_server contains a copy of esaml taken from ? and located in ns_server/deps.  It has been suggested that we update the copy and reapply our customizations. It has also been suggested we consider having a proper fork (e.g. github.com/couchbasedeps) rather than a copy.
