This seemed to be a side effect:

          NullConfigException: NodeLocator is not defined

Bootstrapping for CCCP fails just before and then HTTP Streaming fails later on with:

          Bootstrapping with 1.2.3.4:11210 failed.

          ...

A connection attempt failed because the connected party did not properly respond after a period of time, or established connection failed because connected host has failed to respond.

Based on the cluster map, however, your using CB 6.5 or greater, which uses GCCCP, so SELECT_BUCKET hasn't been called yet and the cluster map was fetched and its the GCCCP cluster map which the client doesn't understand (its SDK2).

          I'll try to replicate...
